These Terms of Use (“Terms”) describe the rules by which you agree to use our websites, patient portals, mobile apps, or other digital services (collectively, the “Insight Services”). By accessing or using the Insight Services, you agree to comply with and be bound by these Terms.
“Insight,” “We,” “Us,” or “Our” refers to Insight Naperville Outpatient Center (and any subsidiaries, affiliated entities, or services we operate).
“You” or “Your” refers to an individual, group, or organization using Insight Services, whether for yourself or on behalf of another.“Insight Services” collectively refers to our websites, patient portals, mobile apps, digital tools, correspondence, reminders, and other related services.

“Content” includes text, images, graphics, data, user interfaces, logos, videos, downloadable files, and other materials provided through Insight Services.
“Your Information” means personal information (identifiable or non-identifiable) you provide or that we collect, including any protected health information (PHI).
You may use the Insight Services for your non-commercial, personal or educational purposes, subject to these Terms.
You agree not to (and you will not assist others to) use the Insight Services in any way that violates applicable law, infringes any third-party rights, or interferes with the operation or security of the services.
You agree not to copy, reproduce, distribute, modify, create derivative works of, decompile, reverse engineer, or otherwise misuse the services or Content except as expressly permitted under these Terms.
If you register for an account (e.g. patient portal login), you are responsible for maintaining the confidentiality of your login credentials.
You agree to notify us immediately if you suspect unauthorized use of your account or any security breach.
We reserve the right to suspend or terminate accounts or access if misuse or security issues are suspected.
Your use of the Insight Services is also governed by our Privacy Policy, which describes how we collect, use, store, and manage your personal information (including PHI). Please review that policy carefully.
The Insight Services are not intended for children under 13 years old.
If you are under the age of majority (as defined in your state), you should use the services only under the supervision of a parent or legal guardian who agrees to be bound by these Terms.
By using the services, you represent that you are at least the age of majority or have legal authority to accept these Terms.
All Content, logos, trademarks, and materials on Insight Services are owned or licensed by Insight.
We grant you a limited, revocable, non-exclusive, non-transferable license to access and use the Content for non-commercial personal or educational purposes, if you comply with these Terms.
You may download or print portions of the Content for your personal use—provided that you retain all copyright and proprietary notices. Any other use requires our prior written permission.
If you post, upload, or otherwise submit content (“Submission”) through Insight Services (e.g. comments, feedback, forums), you represent that:
You own or have the rights to submit it;
It does not violate any law or third-party rights;
It is truthful and not misleading.
By making a Submission, you grant Insight a worldwide, royalty-free, perpetual, sublicensable license to use, reproduce, modify, display, distribute, and otherwise exploit the Submission, along with your name, city, or other identifying information, in connection with Insight Services or other channels.
If we provide SMS, push notification, or other mobile messaging services:
You may be asked to provide consent to receive text messages.
You can opt out (e.g. by replying “STOP” or via your account settings), but some messages (e.g. appointment reminders) may be considered part of your care relationship.
Message and data rates may apply per your mobile plan.
We and our vendors are not responsible for delays or failures in message delivery.
Insight may provide links to external third-party websites or resources. We do not control those sites and are not responsible for their content or practices.
Accessing third-party sites is done at your own risk.
You may link to portions of Insight Services for non-commercial, non-misleading purposes, so long as you do not portray Insight in a false or defamatory way, or associate our Content with objectionable content.
The Insight Services are for educational and informational purposes only and are not intended to constitute medical advice, diagnosis, or treatment.
Use of the services does not create a physician-patient relationship.
Always consult a qualified healthcare provider if you have medical concerns or symptoms.
In the case of emergencies or life-threatening conditions, call 911 or your local emergency services immediately.
To the fullest extent permitted by law, Insight and its officers, employees, agents, and licensors will not be liable for any indirect, incidental, special, consequential, or punitive damages (including lost profits, data loss, or business interruption) arising out of or relating to your use (or inability to use) the Insight Services or Content, even if advised of the possibility of such damages.
Our total aggregate liability in connection with these Terms, the services, or any claim will not exceed the amount you paid (if any) to use the services.
You agree to defend, indemnify, and hold harmless Insight and its affiliates, employees, agents, licensors, and contractors from any claims, damages, losses, liabilities, costs, or expenses (including attorney’s fees) arising out of your use or misuse of the Insight Services, your Submissions, or your violation of these Terms.
These Terms are effective as of your first use of the Insight Services and remain in effect unless terminated.
Insight reserves the right, in its discretion, to suspend, restrict, or terminate your access to all or part of the services at any time, with or without notice, for any reason.
Upon termination, all licenses and rights granted to you will cease, but provisions that by their nature should survive (e.g. indemnification, limitation of liability, intellectual property) will remain in force.
We reserve the right to update or modify these Terms at any time. Revised Terms will become effective when posted. Your continued use of the Insight Services after such posting constitutes your acceptance of the modified Terms. Please check this page periodically.
If any provision of these Terms is found invalid, illegal, or unenforceable by a court of competent jurisdiction, that provision will be enforced to the maximum extent permitted by law, and the remaining provisions will remain in full force.
These Terms and all matters related to your use of the Insight Services will be governed by and construed under the laws of the State of Illinois, without regard to conflict of law principles. Any dispute will be brought in state or federal courts located in DuPage County, Illinois, and you consent to the exclusive jurisdiction and venue of those courts.
If you access the Insight Services from outside the United States, you acknowledge that U.S. law may not provide the same protections as your local jurisdiction. You consent to the transfer and processing of your information in the U.S. in accordance with our Privacy Policy.
To ensure timely care for all our patients, Insight Naperville has implemented a No-Show and Late Cancellation Policy. Appointments that are missed without notice, or canceled within 24 hours of the scheduled time, will incur a $25 fee. This policy helps us manage our schedule efficiently and offer appointment slots to patients who may be waiting for care. We kindly request that all patients notify us as early as possible if they need to reschedule or cancel. Your cooperation allows our team to provide consistent, high-quality service to every patient in our community. Thank you for understanding.
